PHP Proxy
PHP Proxy is also extemely popular however it is no longer being developed. The project abandoned as of September 7th 2007. Similar to CGI Proxy PHP Proxy is extemely easy to use. PHP Proxy has the added bonus that there is many templates which have been developed for it helpful for those who don't want to much around with websites.
